Visits to obtain a MMJ card are by appointment only. Call the Fernandina or Jacksonville office to get scheduled. Remember to bring your ID and pertinent medical records.

Get Added to the Registry

If approved, your doctor will enter your information into the Florida Medical Marijuana Use Registry, an online system maintained by the Florida Department of Health.

You must submit a photo ID, proof of residency, and a $75 application fee.
